Community Health Memberships are non-insurance service agreements designed to provide affordable, predictable access to basic medical care within our clinic. These memberships are structured to support individuals and small businesses seeking convenient access to care without traditional insurance billing.
These programs do not replace insurance and do not provide hospital, emergency, or pharmacy benefit coverage.
Monthly Membership Fee: $25 per month
Service Access Fee: $75 per visit
Lab Charge: $150 per lab panel

Community Diagnostic Referrals
When services are not available within our clinic, we refer patients to trusted community partners offering free or low-cost diagnostic care.
Free Breast & Cervical Cancer Screenings:
Every Woman Counts (EWC) – State-funded mammograms and Pap tests for eligible patients
Valley Breast Care – Dedicated EWC provider
Grand Avenue Imaging – Free screenings for eligible patients and fixed-fee self-pay options
Low-Cost Diagnostic Imaging:
Ultrasound Institute Medical Group (UIMG) – Affordable out-of-pocket ultrasounds
Saban Community Clinic – Sliding fee program based on income
Virgil Family Health Center – Community-based reduced-cost imaging
Alvarado Health Center (LA County DHS) – County network for uninsured care
Mobile Screening Services:
City of Hope Mobile Clinic – Community-based 3D mammography services

Patients with high-deductible PPO plans (e.g., $5,000+ deductible)
Small businesses seeking affordable access to basic care
These memberships are intended to complement, not replace, insurance coverage.

Membership enrollment is available to new and existing patients.

Important Information
Community Health Memberships:
Are non-insurance service agreements
Do not cover hospitalizations, emergency care, specialty care, or pharmacy benefit coverage
Provide services only within our clinic
Do not include coverage outside of Nanoom Medical Group
Must not be combined with Medicare billing or insurance billing for the same services
If a patient is enrolled in Medicare, Nanoom Medical Group will not bill Medicare for services covered under the membership agreement.
Monthly membership fees will not exceed $150 per month in accordance with regulatory guidelines.
A full Membership Service Agreement outlining scope of services, limitations, and terms will be provided prior to enrollment and must be acknowledged before activation.
Community Diagnostic Referrals
For services not provided within our clinic, we may refer patients to community partners, including government and nonprofit organizations offering low-cost or no-cost services when available. A diagnostic referral handout is available upon request.
Enrollment
Community Health Memberships are purchasable online through subscription.
Membership enrollment is available to adults age 18 and older.
